Health Link has been serving the Bay Area community since 2005. Our mission is to provide patients with quality care after a serious illness, injury, or new diagnosis. Licensed clinicians go directly into people’s homes to provide the care they need, as if their own doctor were able to be there. In this way, it acts as an extension of the patient’s physician. Home health care is designed for patients who are 18 or older, have a skilled medical need, and are considered homebound. Being homebound means that leaving home requires a considerable effort or could worsen the patient’s illness, injury, or new diagnosis. This type of care is short-term and intermittent, with the goal of helping patients return to independent living as quickly and safely as possible. Home health services are covered by Medicare and most major insurance plans, and Health Link has one of the highest acceptance rates for commercial insurance. Health Link also provides palliative care, which is available to patients of any age and at any stage of a serious illness. Palliative care can be offered alongside home health services or other treatments, with the primary focus on symptom relief, comfort, and improving quality of life for both patients and families. For patients nearing the end of life, Health Link offers hospice care. To be eligible, patients must have a terminal illness with a life expectancy of six months or less, as determined by their physician. Hospice shifts the focus from curative treatment to comfort and support, prioritizing dignity, peace, and quality of life for both patients and their families.
